  • Treatment recommended or received
  • Relapses
  • Driving while impaired
  • Medical complications
  • Other substance use
  • Occupation
  • Member of a support group
  • History of bankruptcy
  • Criminal history

What response can you expect?

Less than 2 years of abstinence:declined
After 2 to 3 years of abstinence:rated +200 may be offered
After 3 to 5 years of abstinence:rated +100 may be offered
After 5 years of abstinence:standard may be offered
If 1 relapse:generally declined
If more than 1 relapse:declined
If use of other substances:declined for a minimum of 7 years, individual consideration thereafter
