Mild to moderate* level and more than 1 year since the diagnosis:	standard or rated
Other:	declined

*Mild to moderate level: independent in carrying out activities of daily living or minimal support required.

Age 18 and less at time of application:	declined
Over than age 18 at time of application:
Mild to moderate* level and more than 3 years since the diagnosis with a job and stable income:	standard with exclusion and/or rated, with limitation of the waiting period to 90 days
