Depending on the situation:

standard or rated +50 to +100

30 years old or less at the time of application:	declined
Over 30 years old at the time of application:	rated +50 to +100 or declined

40 years old or less at the time of application:	declined
Over 40 years old at the time of application
Within 6 months of the diagnosis:	declined
More than 6 months since the diagnosis:	standard or rated +25 to +100 with waiting period of at least 90 days
