More than 5 years since the last relapse:rated +50

Moderate form

Within 5 years of the last relapse:rated +150
More than 5 years since the last relapse:rated +100

Severe form

Within 10 years of the last relapse:declined
More than 10 years since the last relapse:rated +250 or declined
