A new version of the
Annuity contract and application – Life annuities and term certain annuity form is now available.
The structure of the document was completely revised to improve the client experience. The contractual clauses and related questions are now grouped together in a consistent manner, making it easier to read and understand the contract. It will make it easier for you to fill out the form and explain the different rules and features of the contract to clients. The contract text was also revised to prioritize simple and clear language.
A new section was also added to request tax withholdings. This section only applies to registered and prescribed non-registered annuities.

new version and start using it now. Please note that applications submitted using the old version of the form will no longer be accepted after July 13, 2026.
